2. 3500 PPO & 3500 HSA Plans
   These plans provide 100% coverage for most services after your deductible is met, with highly affordable rates, the 3500 PPO plan also offers a prescription benefits before your annual health deductible.  The 3500 HSA plan qualifies as a high deductible plan if you wish to set up a Health Savings Account (HSA).  These plans do not include maternity coverage.

4. Lumenos HSA:  This plan offers you a Health Savings Account compatible plan to work alongside an HSA that can be funded by your own pre-tax contributions.  It also included a Traditional Health Coverage component – similar to a typical health plan – which helps protect you against further health expenses.
5. Dental Blue:  Something to Smile About – No matter which Dental Blue plan you choose, you can see a dentist or specialist in any of the Dental Blue networks and still be considered “in-network.”
6. HMO Plans:  Four HMO plans to choose from.  Predictable out of pocket costs, usually with a low co-pay.  Simply choose an Anthem Blue Cross of California Primary Care physician who will coordinate your health care services and authorize referrals to any specialist you may need.
7. RightPlan PPO 40:  A “No-Deductible” plan, which provides significant savings for you – A $40 co-pay for physician visits, with no annual deductible.  Full major medical benefits along with an immediate coverage $40 co-pay.
8. PPO Share (500, 1500, 2500 & 5000 Deductible plans) Our most comprehensive PPO coverage.  Four deductible options, office visit co-pay, Brand and Generic prescriptions, Inpatient/Outpatient coverage along with Maternity benefits.
