Gerber Medicare Supplement Application
The Gerber Medicare Supplement application at first look may seem like quite a task to complete. Actually the application is designed to be completed rather quickly. It is a one or
two person application, so if yourself and a spouse are applying at the same time, one application is used.
Completing the Gerber Medicare Supplement Application …
- Part 1: Is for the typical personal information such as; name, address, phone, date of birth, Medicare number, email address, male or female, height and weight.
- Part 2: Asks a few general yes/no questions; have you used tobacco in the last twelve months, do you have Medicare part A and B, are you under age 65, did you turn 65 in the last six months.
- Part 3: Is about your current medical coverage. Are you in a guaranteed issue period, do you currently have a Medicare Supplement policy, are you on an employer group plan, are you replacing your current Medicare Supplement, have you had continuous coverage for the past 63 days.
- Part 4: Yes/No medical questions, current prescription drugs.
- Part 5: Read, sign and date.
- Part 4 Cont.: Additional prescription drug information if needed.
- Electronic Funds Transfer: If you choose to pay by automatic bank debit.
- Authorization to disclose personal information to Gerber: Sign and Date. If Gerber needs to request medical records from you doctor, this form is needed.
- Replacement form: Sign and date only if replacing an existing Medicare Supplement plan.
- Open Enrollment form: Sign and date. Outlines the open enrollment requirements.
That’s it. Not as bad as it looks. The application should take less than 30 minutes to complete, unless there is detailed medical and prescription information, in which case may take a little longer. If paying by automatic debit, you can fax the application to: 800-995-9913, for initial review. If the Gerber application is completed correctly it will then be faxed to the Gerber Medicare Supplement processing department to be input,�reviewed and issued.

Processing time can take up to four weeks. All Gerber Medicare Supplement applications are processed by Mutual of Omaha. Gerber has contracted Mutual of Omaha to handle much of the administration of the Gerber Medicare Supplement policy. So if you are in a hurry, you should be patient and be confident the processing will be completed in the order it was received.

Does the $25 application fee for Gerber Med Supps apply in California?
Hi Travis, yes unfortunately there is no way of getting around the Gerber $25 application fee. It’s in all states which Gerber offers Med Supp plans.
