• Part 1:  Is for the typical personal information such as; name, address, phone, date of birth, Medicare number, email address, male or female, height and weight.
• Part 2:  Asks a few general yes/no questions; have you used tobacco in the last twelve months, do you have Medicare part A and B, are you under age 65, did you turn 65 in the last six months.
• Part 3:  Is about your current medical coverage.  Are you in a guaranteed issue period, do you currently have a Medicare Supplement policy, are you on an employer group plan, are you replacing your current Medicare Supplement, have you had continuous coverage for the past 63 days. 
• Part 4: Yes/No medical questions, current prescription drugs.
• Part 5:  Read, sign and date.
• Part 4 Cont.:  Additional prescription drug information if needed.
• Electronic Funds Transfer:  If you choose to pay by automatic bank debit.
• Authorization to disclose personal information to Gerber:  Sign and Date.  If Gerber needs to request medical records from you doctor, this form is needed.
• Replacement form:  Sign and date only if replacing an existing Medicare Supplement plan.
• Open Enrollment form:  Sign and date.  Outlines the open enrollment requirements.

That’s it.  Not as bad as it looks.  The application should take less than 30 minutes to complete, unless there is detailed medical and prescription information, in which case may take a little longer.  If paying by automatic debit, you can fax the application to: 800-995-9913, for initial review.  If the Gerber application is completed correctly it will then be faxed to the Gerber Medicare Supplement processing department to be input, reviewed and issued.

Processing time can take up to four weeks.  All Gerber Medicare Supplement applications are processed by Mutual of Omaha.  Gerber has contracted Mutual of Omaha to handle much of the administration of the Gerber Medicare Supplement policy.  So if you are in a hurry, you should be patient and be confident the processing will be completed in the order it was received.

The Gerber Medicare Supplement plans are raising many eyebrows with their competitive rates and solid name recognition because of the baby food and life insurance devisions.  If comparing Medicare Supplement plans be sure to include Gerber in your comparison.  You may be surprised at what you see.

Medicare Supplement plans shield you from the unexpected.

Gerber Medicare Supplement plans are offered in many states.  Plans A, F and G are available in most, along with plan C in a few states.  F is the most comprehensive of the three plans offered by Gerber.  Deductibles, coinsurance and out of pocket costs are covered by your Medigap F plan.  Having a comprehensive benefit plan like plan F allows you to budget and plan for other expenses throughout the year.  You are not surprised with a large medical bill as you may be with other Medicare Supplement plans.  The only out of pocket expenses you will see are the monthly premiums, which you can budget for.

Remember; with a Gerber Medicare Supplement plans, you choose:

• Any doctor, specialist or hospital that is Medicare approved.
• Where you want to receive care or treatment anywhere in the U.S.
• The plan that provides the benefits you need.
• Leave the paperwork to the doctor and us.

Your Gerber policy is guaranteed renewable.  It cannot be canceled.  It will be renewed as long as the premiums are paid on time.  Your Gerber Medicare Supplement benefits will automatically increase as Medicare deductibles and coinsurance increase.  Benefits are not paid for any expense paid by Medicare.  You will have 31 days from your renewal date to pay your premium.  Your policy will stay in force during this 31 day grace period.  You cannot be singled out for a rate increase, no matter how many times you receive benefits.  Your coverage begins immediately.  There are no waiting periods for preexisting conditions.  Benefits will be paid from the time your policy is in force.

Premium payment options:

1. Option A: Pay all premiums (1st and monthly payment) by Electronic Funds Transfer, which is an automatic Bank or Checking account deduction.
2. Option B: Pay the first month by check and monthly payments by Bank account deduction.
3. Option C: Pay the first month by Bank account deduction, then pay by check.  Paying by check (quarterly, semiannual or annual only).

Applying is Easy: Simply call or email for an application and fax or mail the completed application with a copy of your Medicare card (Red, White and Blue card).  The process usually takes a couple weeks for approval.  Once approved a Gerber Medicare Supplement policy along with your new ID card will be mailed to your preferred address (residence or mailing address).

You need to know the difference between the different Medicare Supplement (Medigap) plans.  Medigap plans are insurance policies sold by private insurance companies that are designed specifically to fill in the gaps left by Medicare.  Medicare Supplement plans help pay for some of the Medicare deductibles and coinsurance which you would be responsible for.  Each plan; A, C, D, F, G, N and M are some of the available California Medigap plans.  Each offers different benefits and all are priced differently.

Each company sets their own rates, so please compare the different rates in your area.  Prices vary drastically.  Also be sure to check the new Medicare Supplement plans M and N.  They offer some unique benefits.  Plan F continues to be the plan of choice for most Medicare beneficiaries because of the comprehensive benefits.

The Medicare deductibles continue to rise, the 2010 part A deductible is now $1,100 per 60 day benefit period.  This is not a traditional annualized deductible.  There are six potential benefit periods per year, meaning there is a possibility you would be responsible for six, $1,100 deductibles in any given year.  This is why most seniors choose to purchase this insurance option called a Medicare Supplement plan.

You will also need to purchase a Medicare Part D Prescription Drug plan.  Most plans are between $30 to $45 per month which provides coverage for your prescription medications.

Common Terms:

• Birthday Rule - (Unique in only a few States) allows a current Medicare Supplement insured to switch to another company each year on the month of their birthday to a plan will equal or less benefits.  This switch is guaranteed regardless of the current or past health issues.  In other word you cannot be denied because of your health.
• Original Medicare - Run by the Federal government and provides both Part A and Part B coverage.
• Medicare Part A - is the hospital coverage Medicare pays for.  You are responsible for the $1,100 benefit period deductible (60 day) when you are hospitalized.
• Medicare Part B- the out-patient coverage Medicare pays for (physician, specialist and surgery services).  There is a $155 annual deductible you need to pay the beginning of  each calender year when you see a physician.
• Part B Premium - All Medicare beneficiaries are required to pay for their Part B coverage.  In 2010 the monthly premium will stay at $96.40 for because of the “Hold-Harmless” provisions in current law.  New Part B members will see a premium of $110.50 for the current year 2010.  Higher income individual and couple rules apply, and may see higher premiums.
• Assignment- an arrangement whereby a physician or hospital agrees to accept the Medicare -approved amount as full payment for services and supplies covered under Part B.  Medicare usually pays 80% of the approved amount directly to the physicians after the beneficiary satisfies the Part B deductible of $155.00.  The Medicare Supplement (Medigap) member pays the other 20%.
• Skilled Nursing Facility - (Medicare Part A) A medical care facility used primarily for rehabilitation.  Patients are typically in a Skilled Nursing Facility when they are recuperating from an accident, illness or surgical procedure.  Medicare typically pays all costs except $137.50 per day.  Medicare will not continue to pay for this service if the person has a degenerative condition.  Meaning, the patients condition should be improving, if not, the coverage is stopped and a Long Term Care policy or Medicaid is necessary to continue to pay for these services.
• Medicare Part D - Medicare Prescription Drug Coverage.  Helps cover the costs of prescription drugs.  Must be purchased from a private insurance company.
• Part B Coinsurance - After the Part B deductible.  Medicare requires you to pay 20% of all Medicare eligible expenses for physician, specialist, ambulance and hospital outpatient services and supplies.
• Excess Charges- When your medical bill for Part B services exceeds the Medicare eligible expense.  For example, if the Medicare allowable charges for certain visit or procedure is $100, Medicare Part B pays $80 and the Medicare Supplement (Medigap) pays the remaining $20, if the Medicare Supplement plan pays for “Excess Charges.  Some plans do not cover this extra charge.
• Initial Enrollment Period - (IEP) Your enrollment is guaranteed if you apply for coverage before or within six month of enrolling in Medicare Part B.  Three months before, the month of, and three months after your Part B effective date.

With a Medicare Supplement Plan F High Deductible Medicare Supplement plan, Anthem Blue Cross and Blue Shield will pay for some or all of the following: 

• Medicare Part A and Part B deductibles
• The 20 percent coinsurance after the Part B deductible.
• Excess fees for physicians who take Medicare but don’t accept the Medicare approved amounts as full payment.
• Health care from any doctor or hospital in the United States who is a Medicare approved provider.
• Emergency health care you may need if you travel outside the United States.

Anthem Medicare Supplement Plans shield you from the unexpected- Without a Medicare Supplement plan, sometimes referred to as a Medigap plan, health care costs can quickly add up to thousands of dollars.

Upon reaching Medicare-eligibility, many people have found that while Medicare provides basic health care coverage, it’s not always enough.

• Medicare Part A provides hospital insurance, yet it does not cover the benefit period deductible or the coinsurance required for hospital care.
• Medicare Part B has an annual deductible and typically covers only 80 percent of Medicare’s approved amount for medical care.

If you are a policyholder of one of the eliminated Anthem plans, you will be allowed to hold onto your current plan for as long as you like.  Or you can transfer to one of the new “modernized” Medicare Supplement plans, M or N, as long as you transfer during the guaranteed issue time period.  If you wish to transfer to one of the other modernized plans such as Plan F you will have to be approved medically by answering certain question on the application.

Changes Effective 6/1/2010:

• Plan G will have 100 percent coverage for part B excess charges (currently at 80%).
• Plan E will be eliminated because it will be the same as Plan D.
• Plans H and I will be cancelled because of the elimination of the prescription drug benefit.
• Plan J will be eliminated because if will be the same as Plan F.
• New Plan M: 50% coverage for the Medicare Part A deductible of $1,100.  No coverage for the Part B deductible of $155.
• New Plan N: 100% coverage for the Medicare Part A deductible, no coverage for the Part B deductible.  After the Part B deductible is satisfied members will have a $20 copay for doctor visits.

San Francisco area Medicare Supplement plans are “Standardized” meaning each company which offers a Medigap Plan F, for example, must all have the same benefits and plan structure.  The only different with the plan is the monthly premium.  Each company chooses how to price a particular Medicare Supplement plan depending on the State, County, Zip code and the age of the person.  Some companies will also provide ‘tobacco’ rates which are higher than the typical rate for that age group.

On June 1 2010 the Medicare Supplement industry is going through a few changes.  There will be two new Medigap plans introduced: Plans M and N.  Also hospice benefits will be added to all plans, and Plans E, H, I and J will be discontinued.  If you currently have one of these plans you can keep them.  New (E, H, I and J) plans will not be issued after June 1, 2010. 

With Hospitals such as, St. Mary’s Hospital, San Francisco General, Chinese Hospital, UCSF, Mt. Zion, Seton Medical Center and many other providers to choose from.  Remember as a Medicare Supplement policy holder, you are entitled to see any physician, hospital or specialist in the United States who accept Medicare.

Enrolling in one of these plans is easy.  Simply complete the enrollment application and fax it to me, I’ll do the rest.  Most companies allow you to fax the completed application so you don’t have to send a check with the app.  Usually the debit form or credit card info is necessary to initiate the payment process.  California residents have two guaranteed issue times.  They have the Open Enrollment which is for people who are turning 65 or new to Medicare Part B coverage, in which case they have three months prior to their Part B coverage starting and three months after, to choose and enroll into a Medigap plan.  During this time your enrollment and acceptance is guaranteed, regardless of your current or previous health status.  California residents also have the “Birthday Rule” which is also a guaranteed issue event.  This allow you to change companies each year on the month of your birthday.  So if a company you choose falls out of favor with you, for whatever reason, you can switch companies during your birthday month, guaranteed.

What is Medicare Parts A and B?  Medicare is a Federal health insurance program that covers individuals who are over age 65 or younger with certain disabilities.

In order to purchase a Medicare Supplement or Medicare Advantage plan, one must first have in place both Medicare Parts A and B.  You will pay a monthly premium for your Part B coverage, along with the Medicare Supplement plan or Medicare Advantage plan.  Medigap plans are individual policies, so if your spouse will also have to apply for a policy.

As we all know, Medicare alone leaves many gaps in coverage for both inpatient care along with outpatient coverage.  This is why so many seniors purchase a Medicare plan to cover the expenses Medicare doesn’t cover.  Medicare Supplement plans C, D, F and G are the most popular of all Medigap plans, with F being the most comprehensive.  Please see the benefit chart below.  A very popular Medicare Advantage (Medicare PPO plan) is the Anthem Freedom Blue plan which is a Medicare PPO plan.  This plan is a zero ($0) premium plan which includes your Part D prescription drug coverage.  The Freedom Blue plan, as popular as it is, has deductibles and copays which you are responsible for throughout the year.  Whichever type of plan you choose, you will be much more secure than having Medicare alone.

 Guaranteed Acceptance:

Your acceptance is guaranteed if you apply for coverage before or within six months of your initial enrollment in Medicare Part B.  If you’ve been enrolled in Medicare Part B for more than six months, we’ll have to review your health history to see if you are eligible for the plan you selected.  (The State of California also has the “Birthday Rule” which is also guaranteed acceptance).

About Pre-Existing Conditions- If you have had at least six months of prior creditable coverage or are in a guaranteed issue situation, you don’t have to wait for coverage to start for a pre-existing condition.  Many types of health care coverage count as creditable coverage, but they only count if you did not have a bread for more than 63 days.  This prior coverage can be used to eliminate or shorten waiting periods for pre-existing conditions.  A pre-existing condition is a condition either treated or diagnosed six months prior to the effective date of your policy.

• Compare Fresno Medicare Supplement Rates

What is the Medicare Supplement ”Birthday Rule?”

If a person already has a Medicare Supplement plan, they have 30 days of “open enrollment” following their birthday each year when they may buy a new Medicare Supplement policy without a medical screening or a new waiting period.  The new policy must have the same or lesser benefits as the old policy.  This rule is only for Medicare Supplement (Medigap) policies.  Medicare Advantage plans (HMO & PPO) have different rules that apply to their open enrollment options.

The two best times to purchase a Medicare Supplement policy is during the “Initial Enrollment Period” which is the six month window when you are new to Medicare Part B, or when you leave your employer health insurance plan.  The other time is during the Medicare Supplement ”Birthday Rule.”  both are Guaranteed issue is situations.  You can also choose not to wait for your birthday and apply for a new company at anytime during the year.  In this situation you will be asked to complete the “Health Questions” on the application, and your approval is based on your current health status.  This is also a very common event.  If you have an ongoing health condition you can apply for coverage with a different company outside of the birthday rule, simply to see if the company will approve you.  Otherwise during the month of your birthday is your best option for lowering your monthly premium.

Currently there are twelve federally standardized Medicare Supplement plan available nationwide.  After June 1, 2010 there will be ten Modernized plans available, four will be discontinued (E,H,I & J), and two new plans will be introduced (M & N).  All plans have different levels of coverage, so please review the benefits before you settle on a plan.  Feel free to call me anytime if you would like to discuss the different plans.

The popular San Diego County Medicare Supplement plan will most likely continue to be D, F and G, simply because of their comprehensive benefit design.

So if you are new to Medicare Part B or your birthday is coming up, you will have guaranteed coverage for a new plan or if you would like to change to company with lower premiums.  If you are new to Medicare Part B you have a 6 month window to choose a Medicare Supplement (Medigap) plan.  The window is three months prior to the month of your birthday and three months after your birthday.  This gives you plenty of time to make the right decisions.  If you are during the month of your birthday (birthday rule - California) you have 30 days following your birthday to change to a new Medicare Supplement company with the same plan you currently have or to a plan with less benefits.  These are both guarantee issue events.

All San Diego County Medicare Supplement companies do not necisarilly offer all 10 Modernized plans.  Some companies will offer 3 plans while other may offer 7, 8, 9 or all ten.  So check with the different company’s to find the plan which fits your unique needs and budget.  You can do this by clicking on the company links above to view the plans offered and the rates accordingly.  You will also need to purchase a Medicare Part D Rx plan to cover your prescription needs.  Most plans will will run between $30 to $45.  Depending on if the plan includes a prescription deductible or not.

Each company will have different rates within Los Angeles County.  The benefits will all be the same for the chosen plan, because the plans are federally standardized, meaning if an insurance company offers a Medicare Plan F for example, the benefits must follow national guidelines.  So any company offering a Plan F or any other Medicare Supplement plan will have the same coverage benefits as any other company.  The only difference will be the premium amounts, company rating and customer service.  All three of these can vary substantially from company to company.

Choosing a Medicare Supplement policy can get complicated because all plans have different levels of coverage.  Plan F being the most comprehensive plan available to Los Angeles Medicare Supplement members.  As a resource, this website is meant to be an information site regarding your Medicare Supplement and Health Insurance needs.  Feel free to view the different pages throughout the site.  If you would like someone to speak to, I am always available to answer your questions.

Medicare Supplement plans have been standardized since 1992 (plan benefits are the same regardless of the company).  All Los Angeles County Medicare Supplement plans will become “Modernized” effective June 1, 2010.  Companies will be able to only sell the new Modernized Medicare Supplement plan effective June 1st.  All current Medigap members will be able to keep their current plans and will be able to transfer to the new plans if they choose to.  All new business beginning June 1st will have to be the Modernized plans.  Two new plans will be introduced; M and N.  I’m sure Plan F and G will continue to be the most popular, simply because of their comprehensive coverage.  After June 1st there will be 10 Medicare Supplement plans available in California.

Remember, regardless of the Los Angeles County Medicare Supplement plan you choose:

• Freedom to choose your own doctor or hospital.
• No required medical exam.
• Guaranteed renewable.

Important Information about a Medicare Supplement Policy:

• You need to have Medicare Part A and Part B.
• You will need to pay a monthly premium with a Medicare Supplement (Medigap) policy.
• Your policy cannot be cancelled, even because of health reasons, as long as you pay your premium on time.
• Each policy covers one person, so you and your spouse will have to have a policy each.
