Anthem Blue Cross Medicare Supplement Application

If you currently have a Medicare Supplement plan with Anthem Blue Cross or any other company, and wish to complete a new Anthem Blue Cross Medicare Supplement application, because you are changing to Anthem, or simply updating to the new Modernized Medicare Supplement policies which became effective 6/1/2010, you can access your states Anthem applications below.  I have also included the monthly premiums for each state for your convenience.

Anthem is an easy company to apply to; simply print your appropriate state application and fax it to: 800-995-9913 for review.  A check is not necessary with the application.

The New Anthem Blue Cross “Modernized” Medicare Supplement plan N is now available to any new enrollee along with Anthem’s current Medicare Supplement customer who wishes to update their current policy to the new Plan N.

One stop shopping with the Anthem Blue Cross Medicare Supplement plans, Part D Rx plans, Freedom Blue Medicare Advantage PPO plan (with Rx), and the Anthem Senior Dental Plans for individuals over age 65.

As of June 1, 2010 the current Anthem Medicare Supplement plans referred to as “standardized” plans will no longer be open for new sales or new membership, Medicare beneficiaries who are currently enrolled in a “standardized” or “pre-standardized” Medicare Supplement plan can retain their current plans as long as their premium payments are current.  Or, you can switch to the new “Modernized” plans.

Anthem Medicare Supplement applications are available in the following states: California, Colorado, Connecticut, Georgia, Indiana, Kentucky, Maine, Missouri, Nevada, Ohio and Virginia.  Please click the appropriate link above to print an Anthem Medicare Supplement application.

To Apply …

If you are new to Medicare or changing companies, simply print the appropriate application for your state (call me and I can walk you through it).  Then when finished simply fax or mail the application back to me.  I’ll do the rest to make sure you receive your requested effective date, and that the enrollment process is quick and accurate.
