The Anthem Dental Blue PPO and Dental HMO plans offer comprehensive dental benefits for individuals and families.  As an Anthem Dental Blue member you will automatically receive your services at a discounted rate.  By showing your ID card the procedures are discounted even before your Dental Blue insurance plan pays it’s portion.

Your savings don’t end there – Save during waiting periods and even after you have reached your annual maximum.  Even if you reach your annual maximum for the year, you will save because you pay the negotiated fee instead of the dentist’s full price for dental services.  The same applies during any waiting periods you may have under your plan or for non-covered services such as porcelain veneers, dental implants, TMJ and orthodontics.  As long as you are a Dental Blue member, the dentists and specialists in the Dental Blue networks will extend the special rates we have negotiated with them to you.

How the Dental Blue Plans work:

You have two great plans to choose from; Dental Blue Basic and Dental Blue Enhanced.  The Basic plan provides coverage for many diagnostic services and preventive care such as cleanings, exams and X-rays as well as fillings, with an annual maximum of $500.  The Enhanced plan includes cleanings, exams, X-rays and fillings, plus certain major services like root canals, periodontal procedures and crowns, with an annual maximum of $1,250.  It also includes orthodontic coverage for children after a 12-month waiting period.

As a Dental Blue member, you can seen any dentist you want, however, you do have the potential for lower costs when you choose a dentist in the Dental Blue 100 network.  Because in-network dentists have agreed to accept our fee schedule for services rendered.  If you choose to go to a provider outside of the Dental Blue 100 network, you can be billed the difference between what we pay our in-network dentists and what your chosen dentist wishes to charge.  But with more than 18,000 California dentists in our Dental Blue 100 network, it’s likely your dentist is part of our network.

How to Apply:  You can apply to one of the Anthem Blue Cross dental plan as a “stand-alone” plan (by itself).  You do not need an Anthem health insurance plan to apply.  You may also request a future effective date, as long as it is after your signature date.  To request a future effective date, simply write “REQUESTED EFFECTIVE DATE XX/XX/2010, in the top margin of the application.  All applications must be faxed to us at: 800-995-9913.  The processing time takes 3 to 5 business days.  Once you are enrolled you will receive a policy and ID cards in the mail.
